Job Summary

 

The Health Management Registered Nurse will assess and engage members for health management, maternal health and wellness program(s). This includes planning, coordinating, implementing, monitoring and evaluating the medical services required to meet the complex health needs of members enrolled in the AmeriBen Medical Management program in order to maximize each member's capacity for self-care, to cost-effectively achieve desired clinical outcomes, and to enhance quality of medical care.

 

 

 

Key Result Areas

 

  • Establishes contact and rapport with patients and family members.

  • Assesses the patient's current status, prognosis, future care needs, treatment plan and alternative treatment plans.

  • Coordinates care with Case Management and Utilization Management staff.

  • Coordinates interventions with the members' physicians.

  • Develops and implements individualized member care plans.

  • Evaluates effectiveness of care plan and progress made by member.

  • Documents findings and appropriate cost effective recommendations.

  • Captures and records any cost savings.

  • Acts as a liaison between referral source, physician and member.

  • Maintains confidentiality of records and information.

  • Maintains and report applicable statistics regarding programs and patient services.

  • Documents ongoing education and engagement.

  • Attends weekly on-site team meetings and monthly staff meetings.

  • Performs all duties within the scope of her/his licensure.

  • Performs other duties as assigned or needed.

 

 

Experience and Skills:

Minimum Qualifications

 

  • Current unrestricted RN license.

  • Associates Degree in Nursing (Bachelor Degree is a plus).

  • Clinical experience in Health Management, Wellness Coaching, and/or Maternal Health preferred.

  • Varied clinical experience, as well as a general understanding of clinical program design, implementation and management/monitoring to support choice in consumer medical care.

  • Knowledge of medical/health insurance terminology, ICD-9/ICD-10, HIPAA and DOL regulations.

  • Strong interpersonal relationship skills and the ability to work effectively with a diverse patient population.

  • Ability to communicate effectively, both verbally and in writing, medical information to health care professionals, employer groups, Brokers, stop loss personnel, internal customers, members and families.

  • Ability to work independently under general instruction and with a team.

  • Time management, organizational, research, analytical, and documentation skills.

  • Experience using a video-conferencing system.

  • Types 45 wpm, 10-key and computer knowledge.

  • Able to demonstrate excellent attendance and punctuality.

  • Excellent customer service attitude and professionalism.

  • Demonstrate Accuracy, Customer Service, Timely Turn-Around (ACT).

  • Exceptional verbal and written communication skills.

  • Proficiency using Microsoft Office (Word, Excel, Outlook, etc.).

  • Ability to work efficiently, perform multi-tasking, prioritize tasks, and meet tight deadlines.

  • Possess a high degree of integrity and discretion, as well as the ability to adhere to both company policies and best practices.

  • Ensure compliance with security practices and procedures, including HIPAA and HITRUST standards.

  • Ability to efficiently and effectively perform the Key Result Areas with or without a reasonable accommodation without posing a direct safety threat to others or self.
